You need to include information regarding what orginization (or person) is gathering these names/emails, what they are going to do with them etc. Also on a technical note, the country scroll bar that appears after you enter your name pops up and hides under whatever is above the widget on the page, I found it impossible to move up the list beyond the city it defaulted after entering my name.
