If Show Local Results is selected, then you need to provide an address, city and state or zip.
Web Results:

Show results from Google Web Search.
Web Sites To Search:

If showing Web Results, you can limit the results to a certain website. Example: cnn.com or cnet.com.
Video Results:

Show results from Google Video Search.
Show Blog Results:

Show results from Google Blog Search.
Blog Sites To Search:

If showing Blog Results, you can limit the results to a certain website. Example: blogspot.com or myblog.com.
Display Mode:

Display results in a vertical list, or in tabs.
